Healthy Expectations is Rockford Health Plans HMO’s prenatal program. As soon as you know you are pregnant, you should call Rockford Health Plans to enroll in Healthy Expectations. The prenatal nurse will monitor your progress and encourage you to eat well, not smoke or drink alcohol, exercise safely, see your doctor regularly, and attend prenatal classes. You will also receive helpful information about pregnancy. If you see your doctor in your first trimester, keep your appointments, and complete a survey that will be sent to you, you will receive a $50 U.S. savings bond for your newborn baby. Will you help ensure that my baby is properly immunized?
Every new baby who is covered by Rockford Health Plans HMO is automatically enrolled in the Childhood Immunization Program. The program helps ensure that all children covered by Rockford Health Plans HMO have the opportunity to be properly immunized. You will receive a postcard from Rockford Health Plans HMO one month before your child is due to receive an immunization, to remind you to make an appointment with the child’s physician. It is important that you follow through and assure that your child receives all recommended immunizations. Rockford Health Plans also communicates extensively with physicians with reminders regarding the required and covered immunizations for children in their first two years.

I want to stop smoking. Can Rockford Health Plans HMO help me?
If you are ready to quit smoking, congratulations! Contact your primary care physician. Your doctor can discuss the available smoking cessation programs with you. Rockford Health Plans HMO will reimburse you up to $50 when you complete an approved smoking cessation program. You can find a list of approved programs in the new member edition of Health Journal, or call the member services department for a list. Member services can also provide you with the reimbursement form.

Yes. Each spring, Rockford Health Plans HMO offers discount coupons worth $10 toward the purchase of bicycle helmets for all members. The coupons can be redeemed at many stores in the area. You will find a list of participating stores in the spring issue of Health Journal or by calling the member services department. Ask for the coupons at the participating stores.

My daughter has asthma. How will you manage her care?
Your daughter’s actual health care will be provided or coordinated by her primary care physician. In addition, Rockford Health Plans has registered nursing staff specifically dedicated to assisting and educating members with asthma to help them successfully manage their health. A member services representative can help you and your child access the program.

I have diabetes and must monitor my blood sugar. Will that be covered?
All Rockford Health Plans HMO members who have diabetes and are required to monitor their blood sugar receive a blood glucose monitor. Members with diabetes are also enrolled in the Diabetes Health Management Program, which is conducted by specially trained registered nurses to help members with diabetes manage their health. A member services representative can help you access the program.
